In reviewing the recent medical literature, the following facts have
been reported related to sciatica and back pain:

I hope these facts answer some of your questions and dispel some of
the common misconceptions about back pain. So with that, lets
take a closer look at how to best treat your back pain and have you
start enjoying your life again.
But, before we get into the particulars of the treatment process, I
think it’s important for you to understand that medical research has
indicated that most treatment options
are literally…
Doomed From The Start
In a review of the scientific evidence of the causes, diagnosis, and
treatment of back pain published by the Swedish Council on Technology
Assessment in Health Care (SBU), medical researchers, Alf Nachemson,
MD, PhD and Eggon Jonsson, PhD, cite three reasons why your efforts to
end your sciatica
or back pain may be “doomed” to failure right from the start.
Reason 1:
X-rays,
MRIs, and CT scans only tell part of
your back pain story. Unfortunately, most doctors aren't trained to
determine the reason why the structures of your spine look the way they
do. Consequently, a diagnosis based entirely on the imaging studies of
your back is usually made. This means that the underlying cause of your
back pain often times will go undiagnosed and untreated.
The reason doctors rely so heavily on imaging studies is
because fewer than 50% of the medical schools train their students how
to perform a systematic musculoskeletal exam, much less a comprehensive
spinal assessment. That’s why most doctors just prescribe medications
or refer you to other specialists. The most you can hope for when going
to a medical doctor is a little bit of short-term relief.
Reason 2:
In a
comprehensive
review of conservative treatment methods (including massage, pain
relief ointments or creams, nonspecific exercise, ultrasound,
electrical stimulation, moist heat, traction, acupuncture, etc.) for
chronic lower back pain, none were shown to have any lasting effect on
back pain.
These treatment approaches address only the symptoms and
ignore the underlying cause of the problem.
Reason 3:
Most back
pain treatment programs are
“generic” meaning they're the same for everybody. You know what I’m
talking about. You go to the doctor or physical therapist and you’re
given a sheet of exercises like the one you see to the left.
Look, if you had high cholesterol, would your doctor hand you a bulb of
garlic and tell you to eat a clove a day for 30 days? Of course not!
You would receive a prescription of something like Lipitor with precise
instructions of how and when to take it. So how can you expect for your
back to feel better if the only thing you do are "over the
counter exercises"?
This approach simply doesn’t work yet sadly it’s the standard of care
for most medical practitioners. Doesn't it make much
more sense to treat your back pain with a detailed program that
addresses the cause of the problem and not the symptoms?
But how could you have known any of this? You just want your back to
stop hurting and you trust the “experts” to guide and advise you,
right?

medical research reports: Back pain treatment is doomed from the start.

Back Pain Treatment
Efforts Are Doomed To Failure, Is There Any Hope For
My Particular Condition?”
Actually there is. Let me explain…
What you’re looking at here is a diagram of the cascading events that
take place in the typical degenerative process.
So starting at the top, there is some type of minor trauma or
repetitive stress that leads to pain and disuse.
Over time, this results in adaptive tissue changes. In other
words, some tissues shorten or tighten up and some tissues weaken or
lose their strength. These are the muscle imbalances that you’ll
hear me talk about.
As time goes on, these soft tissue changes will eventually result in
structural changes. That’s where we see the arthritic joint
changes, disc herniation, stenosis, etc
And along with these changes, there is a significant reduction
in mobility, which, over time, results in scar tissue formation and
ultimately leading to chronic pain and disability.
In looking at the most current research regarding the effectiveness of
back pain treatment, the SBU and the International Spine Society have
issued 4
consensus recommendations about sciatica and back pain treatment:
Back Pain Treatment Recommendation #1: Treatment should be geared towards preventing disability and improving the patient’s quality of life. In other words, we need to teach the patient about how to become more self sufficient in the management of his or her own condition and become less reliant on the medical system.
Back Pain Treatment Recommendation #2: Long-term drug therapy is simply not effective in managing back pain and should be avoided.
Back Pain Treatment Recommendation #3: Manual therapy or manipulation is an effective treatment method in reducing pain.
Back Pain Treatment Recommendation #4: Exercise therapy is effective in the restoration of daily function.
Bottom line: The #1 goal in treating sciatica and back
pain is to keep you active.

Once we have a good idea of the location
and likely underlying cause of
your pain, we need to screen for any red flags and assess the behavior
of your symptoms.
So on the left is a typical medical history form that will alert me to any red flags
And on the right is a symptom behavior form that helps me determine how your symptoms respond to different stimuli and creates a time line for me to follow so that I can make some assumptions with regard to cause and effect.

Step 3: Determining The Underlying Cause of the Pain

The key here is a targeted

Here are the results of a study published in Spine Journal a few years
ago that tracked the recurrence of pain in a group of individuals that
had participated in a functional rehabilitation program
Group 1: 100 of the participants were to continue with a maintenance program following the initial functional restoration program
Group 2: The remaining 25 participants were to perform no maintenance program at all.

After 2 years, about 9% of the
maintenance group reported some recurrence in pain while all 25 of the
non-maintenance group reported a recurrence of pain.
This study demonstrates the importance of not only treating back pain
with a Functional Restoration Program, but clearly identifies the
importance of maintaining some form of maintenance following the
initial program.
